Upstate Family Health Center, a Federally Qualified Health Care Center (FQHC) located in the heart of the Mohawk Valley in Utica, NY, is seeking passionate and dedicated Registered Nurses to join our team.

Position: Registered Nurse (RN)

Schedule: 4-day work week, Monday - Friday

Pay: $35 - $45 per hour

Position Overview

As an RN at Upstate Family Health Center, you will play a crucial role in the multidisciplinary process of providing care and treatment to patients. You will provide clinical support and technical assistance to professional, medical, and other staff. Working within a Patient Centered Medical Home (PCMH) team-based care model, you will form effective partnerships with patients, their caregivers/families, and community resources in primary care settings.

Primary Responsibilities

  • Clinical Leadership: Monitor clinical team workflow, delegate tasks as needed, complete paperwork within the RN scope of practice for provider review and sign-off, and tend to patient sick calls.
  • Team Model: Work with a team consisting of 1 MD, 3 NPs, 2 LPNs, and 1 Medical Assistant.

Daily Team Model Clinical Activities

  • Room patients, including taking vitals (Ht, Wt, BP, Temp, Pulse, Resp, O2 Sat) and required questionnaires.
  • Perform Point of Care Testing as ordered by the Provider.
  • Administer office medications as prescribed by Providers.
  • Maintain exam rooms with necessary supplies and materials.
  • Prepare equipment and assist providers during treatment, examination, and testing of patients.
  • Re-enforce patient/family education about diagnostic procedures, nutrition, and health maintenance at the Provider’s request.
  • Prepare patients for physician examination.
  • Screen patients for appropriate information and re-enforce instructions for sample collection and tests.
  • Maintain/review patient records, charts, and other pertinent information.
  • Ensure patient confidentiality.
  • Attend in-service training, team, and other appropriate medical, staff, and health center meetings as delegated by the Senior Director of Clinical Operations.

Universal Duties

  • Attend mandated continuing education/training programs.
  • Comply with all pertinent regulatory and agency regulations and operating standards.
  • Establish a positive work environment that fosters communication between supervisors and employees.
  • Participate in team activities, including meetings, trainings, and committees.
  • Promote and maintain a safe working environment.
  • Perform other duties as assigned.

Required Experience

  • 1 to 2 years of experience preferred but open to new graduates.
  • Unencumbered Registered Nursing License.
  • 1-3 years of experience in a medical office preferred.
  • 1-3 years of experience working with EMR systems and data reporting.
  • Intermediate PC skills (Excel, PowerPoint, Word, Outlook, SharePoint).
  • Ability to act calmly and effectively in emergency situations.
  • Flexibility to respond to changing demands.
  • Exceptional communication and interpersonal skills.

Physical Demands and Work Environment

  • Ability to type and operate a personal computer with various software programs, effectively operate standard office equipment, and perform tasks requiring physical activity (e.g., bending, stooping, crouching, kneeling, lifting up to 60 pounds).
  • Establish and maintain effective work relationships with co-workers and patients, maintain regular attendance, and carry out a variety of oral and written instructions.
  • Certain deadlines and unanticipated developments may require work during evenings and weekends.
  • Ability to address emergent issues without losing focus on tasks at hand.
